Profit Sharing — Contributions to the plan will be determined by the Board of Trustees and Administration on an annual basis. Eligible staff members must be at least 18 years of age and must work a minimum of 1000 hours and be employed on last day of plan year. 401(k) — Employees may elect to contribute some of their own earnings on a tax deferred basis for the purpose of retirement savings. GMC provides a matching contribution up to 2% based on the employee’s contribution. Health Benefits Health, Dental & Vision Insurance — GMC provides Health, Dental, and Vision insurance to all full and part time staff members who work greater than 35 hours per pay period. Coverage begins the first of the month following 90 days of employment. GMC pays the full monthly premium for full-time and a prorated amount for part-time staff based on hours worked. Pharmacy Card Plan is included in health benefit package. Medical Services Discount — For hospital services provided at GMC, and after one full year of continuous employment, a 20% discount will be taken on the balance of the bill (after insurance has paid) for all staff members. Pharmacy Discount — At the completion of an introductory period, staff members may purchase over-the-counter drugs at a discounted rate for themselves as well as their dependents at the Gabert Clinic Pharmacy. Section 125 Flex & Health Reimbursement Account (HRA) Plans Flex — Staff members may elect to set aside money for out-of-pocket medical expenses, family health and dental premiums and/or dependent care in Flexible Spending Account. These amounts are redirected from the employee’s salary before it is subject to Federal and State income taxes and Social Security taxes. HRA — For staff members who participate in the employee sponsored health plan, GMC will fund an annual amount (depending on full or part time status) into an account to offset expenses with the health plan that are applied to annual deductibles and/or co-payments. Long Term Disability Insurance — Paid in full by GMC, this benefit provides disability income in the event the employee becomes disabled resulting is more than 90 days away from work. Employees are eligible the first of the month following 90 days of employment. Eligible employees must work at least 40 hours per pay period to qualify. Term Life Insurance — Employees are eligible for this employer provided insurance benefit the first of the month following 90 days of employment and work 35 or more hours per pay period. This benefit provides $10,000 of Life Insurance coverage. Additional term life insurance coverage is available, should the employee wish to purchase it. Employee Exercise Program — For a fee of $5.00 per month, all staff members may use GMC’s exercise facility on their own time during posted hours. Time Off From Work Paid Time Off (PTO) Bank (hourly accrual is based on a full-time staff member) — PTO hours are earned on all regular and benefit hours paid. Accrual rate for PTO hours: 0 to 4 years (.07692 per hour) = 160 hours per year (320 hour maximum) • 4 or more years (.09615 per hour) = 200 hours per year (400 hour maximum) Extended Illness Bank — Following the completion of the introductory period, extended illness hours are earned on all regular and benefit hours paid. Accrual rate for extended illness hours is .0462 per hour (240 hour maximum). (Hourly maximums and accrual schedules may vary based on job classification.) Bereavement Leave — Bereavement leave is provided for full-time and part-time staff members should a death occur in their immediate family. Full-time staff members are provided up to 32 hours of bereavement leave. Part-time staff members are paid bereavement benefits according to scheduled work days lost not to exceed 32 hours. Jury Duty Leave — Staff members summoned to jury duty will be allowed time away from work without loss of earnings or benefits. (Pay will be regular straight time rate for scheduled hours worked up to 80 hours per pay period.) Additional Benefits Continuing Education — GMC understands the value of additional education to employees. GMC may elect to assist a professional/technical employee with part or all costs related to educational programs in their related field. Supplies Discount — At the completion of their introductory period, (520 hours) staff members may purchase supplies from the Materials Management Department at a discount. Employee Recognition — One staff member will be selected each quarter for a 3-month period. Each staff member recognized as “Employee of the Quarter” will receive: $50 in cash, a special designated parking space, and a display photograph. Jeans for Charity — By donating $1.00 per month to the program, staff members may wear jeans to work on the last Friday of the month. The money goes toward a charitable community cause each month. Student Loans/Loan Repayment – GMC offers financial assistance for qualifying employees in designated fields of study. |
