How to Make a Referral

We accept referrals from:

• Self-referrals (older adults contacting us directly)
• Family members and caregivers
• Primary care providers and specialists
• Hospitals, rehab facilities, home health agencies, and senior living communities
• Community agencies or clergy

To refer someone to Senior Life Solutions:

Our team will conduct a confidential assessment to determine if the program is appropriate and will coordinate with the individual’s current providers.
