  • Allergies

    Level I Office Visit: $95.00/Level II Office Visit $145.00 - Allergies may be seasonal or year round. Common symptoms may include sneezing, a stuffy or runny nose, itchy or watery eyes, and may have an itchy throat or itchy ears. If you experience a flare-up of your asthma, shortness of breath, chest pain, hives, rash, swelling of the throat/ tongue/lips, or difficulty swallowing after these symptoms, please call 911 and visit the GMC Emergency Department. 

  • Arthritis

    Level I Office Visit $95.00 - Arthritis is inflamed or swollen joint(s). Common symptoms may include swelling, pain, and stiffness.

  • Asthma

    Level II Office $145.00/Nebulizer Treatment (If Applicable) $65.00 - Common symptoms of Asthma may include a frequent dry cough, wheezing or mild shortness of breath. Your asthma may become worse with exercise, colds, bronchitis or allergies. If you experience a flare-up of your asthma with severe shortness of breath or chest pain, please call 911 and visit the GMC Emergency Room. 

  • Bladder or Urinary Tract Infection

    Level I Office Visit $95.00/Urine Screen (If Applicable) $22.00 Additional Charges May Apply Upon A Positive Result - Bladder and Urinary Tract Infection symptoms may include urinating frequently, persisting urge to urinate, and pain or burning with urinating. Common causes may include bacterial infection, dehydration, virus, kidney stone, or sexual transmitted infection.

  • Bronchitis

    Level I Office Visit $95.00/Level II Office Visit $145.00/Nebulizer Treatment (If Applicable) $65.00 - Bronchitis symptoms include coughing; sometimes thick mucus causes chest congestion, fatigue, fever and sometimes wheezing. Bronchitis is an inflammation of the bronchus caused by a virus or bacteria. 

  • Cold Sores

    Level I Office Visit $95.00 - Cold Sore symptoms may include a blister or sore on or near the lips or mouth. These are commonly caused by a virus infection.

  • Common Colds

    Level I Office Visit $95.00/Level II Office Visit $145.00 - Common cold symptoms may include a runny or stuffy nose, mild sore throat, cough, nasal congestion, fatigue, low grade fever, ear pressure or pain, and sometimes watery eyes. Common colds can be caused by many types of viruses, although rhinoviruses are the most common culprit.

  • Diarrhea Only

    Level II Office Visit $145.00 - Common symptoms of diarrhea may include frequent loose or watery stools, and often times accompanied by a stomach ache. Diarrhea is often caused by infection from a virus/bacteria/parasite or reaction to food or medication.

  • Ear Aches/Problems

    Level I Office Visit $95.00/Level II Office Visit (Includes Procedure) $145.00 - Ear ache symptoms may include ear itching, pressure, pain, ear drainage, fever, or decreased hearing. Common causes may include a middle or outer ear infection, foreign object in the ear, fluid in the middle ear, or minor injury. This can also be attributed to a cold/respiratory illness or due to recent swimming. Often ear wax may build up in the ear canal. Common symptoms may include poor hearing, a full feeling, itching, tenderness, or irritation. To clear ear wax a therapeutic washing out of wax buildup is used and sometime may need an ear wax softener. If the ear pain is accompanied with redness or swellings of the skin around the outside of the ear or onto the cheek/eye region, or you are not able to open your mouth completely, please visit the GMC Emergency Department.

  • Eye Infections/Injury/Problems

    Level I Office Visit $95.00/Level II Office Visit (Includes Procedure) $145.00 - Eye infection symptoms may include redness, itching, irritation, pain, pressure, fever, swelling, draining/crusted (matting) or tearing, sudden problems seeing. Common causes may include infection such as Pink Eye, sty, dry eyes, allergies, foreign objects causing irritation, or virus. Eye injuries may include eye redness, itching, irritation, pain, pressure, swelling, draining, crusted (matting), or tearing eyes and sometimes sudden problems seeing. Eye injuries are often caused by foreign objects such as dirt, grass, leaves, gravel, fingernail scratches, eyelashes, splashed or spilled liquid that has entered the eye.

  • Gout

    Level II Office Visit $145.00 - Gout symptoms may include pain or swelling/inflammation of joints usually toes/feet, redness, pain with walking/standing or burning/hot feeling. Gout often occurs from a type of arthritis or an increased level of uric acid.

  • Headaches

    Level II Office Visit $145.00 - Headache symptoms may include pain to head, vision changes, congestion, pounding or throbbing, fatigue (tired) or ringing/buzzing in the ears. Common causes may include stress, migraines, dehydration, viruses or bacteria infections, allergies, sinus pressure.

  • Hemorrhoids

    Level I Office Visit $95.00 - Common symptoms of hemorrhoids may include pain with bowel movements, slight bright red blood with bowel movement, swelling/inflammation at or inside the rectum and sometimes pain with sitting or during an activity such as riding a bike. Hemorrhoids are swollen veins inside or outside the rectum or anus and common causes of hemorrhoids include constipation, recent child birth or a diet lacking fiber.

  • Flu/Influenza

    Level I Office Visit $95.00/Level II Office Visit $145.00/Influenza Test (If Applicable) $66.00 - Commonly known as the Influenza virus, influenza symptoms may include coughing, sneezing, runny or stuffy nose, body aches, chills, fevers, irritated or watering eyes, and congestion. We may have to test for influenza virus which will require an additional lab charge.

  • Laryngitis

    Level I Office Visit $95.00 - Common symptoms of laryngitis may include hoarse or loss of voice, sore throat, and discomfort with swallowing, runny or stuffy nose, low grade fever or tender lymph nodes. Common causes may include sore throat, viruses, allergies, frequent coughing or reflux (GERD).

  • Medication Side Effects

    Level II Office Visit $145.00 - Sometimes medications either prescribed or over the counter cause problems or interaction with your body. Common side effects of many medications may include simple allergy reactions such as rash/hives, nausea/vomiting/diarrhea, dizziness, change in mood, body aches, fatigue/ drowsiness (tired), headaches, stomach ache, and constipation.

  • Nose Bleed (Epistaxis)

    Level II Office Visit $145.00/Nasal Packing (If Applicable) $60.00 - Nose bleeds can have many causes and can be mild to serious in severity. It may not be appropriate to be treated in Urgent Care depending on your underlying health problems.

  • Rashes/Minor Skin Problems

    Level I Office Visit $95.00/ Level II Office Visit $145.00/Simple Incision and Drainage (if applicable) + $60.00 - Common symptoms of rashes or minor skin problems may include itching, irritation, redness, pain, mild swelling, blisters, fever, body aches, dry or flaking skin, cracking of skin, pus or draining sores, rough patches or scaly skin. Common causes may include ringworm, chicken pox or shingles, eczema, impetigo, allergic skin reaction, virus, bacteria, yeast infection, sunburn, ingrown hair, foreign object in the skin, poison ivy/oak, psoriasis, dry skin, anxiety reaction, medication side effect, skin color changes or small bruises.

  • Respiratory Infection/Illness

    Level I Office Visit $95/Level II Office Visit $145.00/Nebulizer Treatment (If Applicable) $65.00 - Common symptoms of a respiratory infection may include cough, chest congestion, low grade fever, runny or stuffy nose, and sore throat.

  • Sexually Transmitted Infections (STIs)

    Level II Office Visit $145.00/Urine Screen (If Applicable) $22.00/Pelvic Exam (If Applicable) $60.00 - Common symptoms of STDs may include, itching, irritation, pain, drainage or discharge, cracked or open skin or sores, redness, rash, or blisters on the genitals or surrounding skin. Other symptoms may include fever, body aches, chills, stomach pain, nausea, vomiting or bad odor. Common causes of genital problems may include cha ng or skin irritation, allergic reaction, a bacteria/virus/yeast infection or sometimes sexually transmitted infections.

  • Sinus Infections

    Level I Office Visit $95.00 - Common symptoms of sinus infections may include stuffy and/or runny nose, sinus pressure or pain, post nasal drip, dry or sometimes hacking cough, fevers, headaches. Common causes may include colds, congestion, sinus infections - bacteria or virus, foreign objects in the nose/sinuses, or allergies.

  • Stomach/GI Problems

    Level II Office Visit $145.00 - Common symptoms of stomach GI problems may include an upset stomach or nausea, vomiting, stomach cramps, diarrhea, body aches, chills, fatigue (tired), fever, rash and sometimes dehydration. Common causes may include viruses/bacteria/yeast infections, reaction to food or medications, heartburn/acid reflux (GERD), ulcers, gas, constipation, diverticulitis, appendicitis, irritable bowel syndrome, parasites, dehydration and many more.

  • Sore Throat

    Level I Office Visit $95.00/Strep Test (If Applicable) $60.00 - Sore throat symptoms may include pain with swallowing or talking, enlarged tonsils or tender lymph nodes on the neck, a change in voice or laryngitis, runny or stuffy nose, fever, headache, fatigue (tired), body aches, chills and sometimes stomach ache. Common causes may include an infection of bacteria/yeast/viruses, allergies, frequent coughing, mono, strep throat, post nasal drip, or acid reflux (GERD).

  • Toothache

    Level I Office Visit $95.00 - Common symptoms of a toothache may include pain, trouble chewing, pressure, jaw ache, headache, fever, drainage of pus on the gums, tooth, or mouth; sores in mouth, and swelling in mouth or face. Common causes may include tooth injury, infection, cracked tooth, jaw problems, teeth grinding, or cavities.

  • Yeast Infections

    Level I Office Visit $95.00/Pelvic Exam (If Applicable) $60.00 - Yeast infections are commonly found in the mouth (thrush), on the skin, feet/toes (athletes foot), or genitals. Common symptoms may include, itching, irritation, pain, redness or rash on the skin, and in genitals sometimes white drainage/discharge.
